1) This plan is not insurance.

2) This plan provides discounts with certain health care providers for dental services.

3) This plan does not make payments directly to the providers of dental services.

4) The plan member must pay for all services and receives a discount from those providers who havecontracted with the Smile4Less discount plan.

5) Smile4Less Discount Dental Plan may charge a reasonable processing fee and a periodic charge as loong as the fee is disclosed to the applicant.

6) If the member cancels the membership within the first 30 days after receipt of discount card and other membership materials, the member shall receive a reiumbursment of all periodic charges paid. The return of all periodic charges shall be made within 30 days of the date of the cancellation. 

7) If a Smile4Less plan member cancels membership for any reason other than non-payment of charges by the member, the discounts will no longer apply and member will be responsible for paying regular price for treatment.
